Symptom Tracker App: Rate, Map and Follow Your Symptoms in CareClinic

CareClinic › Getting Started › Track Symptoms

"How bad has it been?" is hard to answer from memory. A quick symptom rating each day turns that question into a clear line you can show your doctor, and it tells you whether a change in treatment is actually helping.

Add the symptoms you feel

Track the symptoms that shape your day. A short list you rate every day tells you more than a long one you skip.

  1. Tap Check-in, then Symptoms to open the Symptom Tracker.
  2. Symptoms from your care plan are already listed. Tap + at the top, or a suggestion under the title, to add one that is not there yet, such as brain fog or nausea.
  3. The free plan tracks up to 5 symptoms at a time. CareClinic Premium removes the limit.

Rate how bad it is

Rate it as it feels right now. Being consistent matters more than being exact.

  1. Tap a symptom and drag the Severity bar. The scale runs from Mild through Moderate to Severe.
  2. Feel the same as last time? Tap Use last value to fill it in.
  3. Tap Save Check-in when you are done.

Mark where it hurts on the Body Map

Where a symptom is can matter as much as how strong it is. A map of your headaches or joint pain is easy for a specialist to read in seconds.

  1. Open a symptom and tap Body Map.
  2. Tap each spot where you feel it. Front Body opens first. Tap Turn for the Back Body, and Face to mark the front or back of your head and face.
  3. Choose the kind of feeling below the map: Pain, Sensory, Movement, Skin, Circulation, Cognitive or Digestive.
  4. Tapped the wrong spot? Tap Remove, then tap the dot you want to delete. Undo brings back the last dot you deleted, and Clear empties the view you are on. Reset at the top clears all four maps after asking you to confirm. Each view holds up to 50 dots.

Watch your Body Map change over time

Pain that moves tells a story. Seeing it travel from your lower back to your hip, or fade over a few weeks of physio, helps you and your care team judge whether treatment is working.

  1. Tap Insights, then Symptoms, and scroll to the Body Map card.
  2. Tap Play to watch your marks day by day. The counter shows the day you are on, and Pause holds it there.
  3. Below, Summary, Active Regions and Symptom Types show where you feel things most and what kind of feeling it is. Body Map Insights is part of CareClinic Premium.

Add a note while you remember

A few words about what you were doing when it started is the detail most people forget by their next appointment.

  1. Under a symptom, tap Notes and write what stood out. On Android notes are free. On iPhone and iPad they are part of CareClinic Premium.
  2. Tag what might be behind it in Factors, like poor sleep or stress.

Tip Rate your main symptom at the same time each day, like after breakfast. Same time, same scale makes the weekly line much easier to trust.

See whether it is easing

After a week or two your ratings become a chart. That is where you see whether the new medication, the physio or the earlier bedtime is making a difference.

  1. Tap Insights, then the Symptoms chip.
  2. Each symptom has its own line. Tap a name in the legend to hide it and focus on the rest.
  3. The Health Insights guide shows how to compare symptoms with your medications.

Common questions

Can I track a symptom that is not on the list?

Yes. On the Symptom Tracker, tap + at the top and type its name. It joins your care plan and is ready to rate every day.

How often should I rate my symptoms?

Once a day is enough for most people. If a symptom changes through the day, rate it in the morning and the evening so the chart shows both.

Is the Body Map part of the free version?

Yes. Marking where you feel a symptom is free. Watching your marks change over time in Body Map Insights is part of CareClinic Premium.

Can my doctor see my symptom history?

You choose. Add your doctor to your Care Team with the sharing you want, or bring a report from the Timeline to your visit.
