CareClinic for Patient Organizations

Give patients something useful between appointments

Offer your community a practical tracking and appointment-preparation resource built around the condition you support. Your team reviews it first, CareClinic maintains it, and members decide whether to use it.

No software integration. No patient list required. No patient health data shared with your organization.

For patients and caregivers

Less remembering. Better prepared conversations.

Living with a condition creates details that are easy to lose between appointments. The resource helps members organize those details without telling them what treatment to choose.

01

Simple daily context

Record symptoms, routines, treatments, and personal observations in one place.

02

Appointment-ready notes

See what changed and write down the questions that matter before a visit.

03

Paper or app

Use the printable plan without installing anything, or use the optional CareClinic experience.

For your organization

Extend your support without adding another program to operate

Your team gets a finished resource it can assess, adapt, and share. CareClinic handles production and maintenance while your organization keeps control of its name and communication channels.

01

Co-branded distribution assets

Receive reviewed web, newsletter, social, QR, and tracked-link materials for your channels.

02

A preview before commitment

Review the member toolkit, app route, privacy boundary, and publishing language before deciding.

03

Useful reporting, clear limits

See campaign activity and resource interest. Patient health information is never part of partner reporting.

A lightweight partnership

From interest to a reviewed pilot

CareClinic does the preparation. Your organization makes the decisions.

1

Tell us who you support

Name the condition, geography, and audience your organization serves.

2

Receive a finished preview

We prepare the member resource, app route, and distribution materials.

3

Review with your team

Approve it, request revisions, test a small pilot, or decline.

4

Your organization chooses the channels and CareClinic maintains the package.

Responsible by design

Trust boundaries your team can explain clearly

The program supports organization and appointment preparation. It does not turn your organization into a health-data processor or ask it to endorse medical advice.

Educational, not diagnostic

Resources help members organize their own observations and questions. They do not diagnose, monitor emergencies, or recommend treatment.

Reviewed before distribution

Your named reviewer sees the exact member resource and publishing copy before anything is shared.

What stays separate

These boundaries remain in every condition package and campaign.

01

No patient health data shared with your organizationPartner reporting is limited to campaign and resource activity.
02

No paid endorsement requirementYour team decides whether the resource is appropriate for its community.
03

No clinical integration requiredThe program works through reviewed resources and public distribution channels.
Everything needed to evaluate it

See the complete resource before your name appears anywhere

Every package is assembled for a specific condition and organization. Your team can inspect the exact experience patients will receive.

01

Versioned and maintainable

Corrections and link changes produce a new version instead of silently changing an approved resource.

02

Ready for the channels you already use

Share it from your site, newsletter, resource directory, events, or social channels.

Common questions

What organizations usually want to know

What does our organization need to provide?

A contact person, the condition and community you support, and feedback on the finished preview. A logo is optional and is never used without approval.

Do we need a technical integration?

No. The standard program uses reviewed resources, tracked links, and your existing communication channels.

Will we receive patient health information?

No. Your organization can receive campaign-level activity reporting, but it does not receive patient health information through this program.

Can members use the resource without the app?

Yes. The member toolkit includes printable tracking and appointment-preparation pages. The app is optional.

Can we review everything before sharing it?

Yes. Your reviewer receives the member resource, condition language, links, and exact publishing copy before making a decision.
