Health Assessments: PHQ-9, GAD-7 and Other Questionnaires in CareClinic

CareClinic › Getting Started › Health Assessments

Questionnaires like the PHQ-9 and GAD-7 are what many doctors use to check how you are doing. Taking one every few weeks in CareClinic gives you a score you can follow over time, instead of a vague sense of better or worse.

Find an assessment

  1. Tap More, then Pathways, then Tests.
  2. Or turn on Assessments in Manage Trackers and open it from Check-in.
  3. Browse groups like For You, Mental Health, Neurological, Pain, Sleep and Geriatric, or use Search Assessments.

PHQ-9

Patient Health Questionnaire for low mood and depression.

GAD-7

Questions about worry and anxiety over the last two weeks.

And more

PTSD and postnatal questionnaires, such as PCL-5 and EPDS.

Take it in a quiet moment

Answer for the period the questionnaire asks about, usually the last two weeks. Your first thought is usually the right answer.

  1. Tap an assessment, then Tap to start this assessment.
  2. Answer each question and finish. CareClinic shows your score with a short explanation.
  3. Tap Post Check-in with Score to save it. If you opened the assessment from Check-in, tap Add to Check-in, then Save Check-in.

Follow your score over time

From memory

"I think I have been a bit less anxious lately."

From your scores

GAD-7 went from 14 to 8 over the six weeks since you started therapy.

  1. Retake the same assessment every two to four weeks, or as your clinician suggests.
  2. Open Insights or Timeline to see your scores side by side.
  3. Bring them to your next appointment, or share a report with your Care Team.

Tip A score is a starting point for a conversation, not a verdict. If a result worries you, talk it over with your doctor or therapist.

Common questions

Can I take the PHQ-9 or GAD-7 in CareClinic?

Yes. Both are in Tests in Health Pathways and in the Assessments tracker. Tap Post Check-in with Score on the result to save it.

How often should I retake an assessment?

Every two to four weeks works for most questionnaires that ask about the last two weeks. Your clinician may suggest a different rhythm.
