A Reminder Is Not a Patient Support Program

The program has an enrollment form, a welcome call and a sequence of refill reminders. On paper, the patient journey is covered.

Then a patient skips treatment because the routine no longer fits around work. Another is unsure whether a new symptom is related to therapy. A third receives the reminder but cannot resolve an access problem. The messages are being delivered. The support is not.

That gap is common in smaller patient support programs. The first version is designed around the actions the sponsor can easily send rather than the problems a patient may actually encounter after starting treatment.

In brief: a patient support program should help an eligible patient move through defined treatment and service milestones. Reminders are useful, but they need to sit inside a workflow that covers onboarding, education, treatment routines, access, patient-reported concerns, safety routing and human follow-up.

Begin with the treatment journey, not the channel

A patient does not experience "SMS," "app" and "nurse call" as separate workstreams. They experience one treatment journey, usually while managing a condition, appointments, family obligations and the uncertainty of a new medication.

The program should therefore start by identifying where people commonly need support:

  • understanding how the program works and who is behind it;
  • completing enrollment and any required authorization;
  • preparing for the first dose or administration;
  • remembering a treatment routine that may change over time;
  • tracking symptoms, treatment experience or function;
  • finding approved education at the moment it is useful;
  • knowing whom to contact about access, product use or clinical concerns;
  • preparing useful information for the next appointment; and
  • continuing support after the initial high-touch period ends.

Some programs also provide benefit investigation, copay assistance, reimbursement navigation, dispensing coordination or nursing services. Those are substantial operational capabilities. A digital patient layer can support them, but it should not pretend to replace them.

This does not require a complex intervention for every patient. It requires a program that can tell the difference between a routine reminder and a signal that somebody may need another kind of help.

Do not call every missed dose "non-adherence"

A digital program can record that a planned action did not occur. It usually cannot know the reason until the patient says so.

A missed or delayed treatment may be related to:

  • a changed schedule or travel;
  • cost, coverage or dispensing delays;
  • difficulty with administration;
  • an unpleasant or worrying experience;
  • a clinician-directed change;
  • forgetfulness;
  • a belief that treatment is not helping;
  • caregiver availability; or
  • a data problem rather than a true missed dose.

The program should record the patient's stated reason when appropriate instead of turning every gap into a motivational message. Better classification can improve service routing and make aggregate program data more interpretable.

How should a patient support program route symptoms and side effects?

Patient support programs are not isolated from pharmacovigilance. FDA's March 2026 final E2D(R1) guidance specifically addresses newer and increasingly used post-approval safety sources, including patient support programs. Read the FDA E2D(R1) guidance.

That does not mean every symptom entry is automatically a reportable adverse event. It means the sponsor should define how information received through the program is reviewed, documented and transferred into the applicable safety process.

Operational rule: the patient-facing app should not decide whether an entry is serious, expected, related or reportable unless that decision is part of a validated, governed process. It can collect and route information; qualified safety personnel own the assessment.

The workflow should specify who receives potential safety information, how quickly it is reviewed, what information is retained, how follow-up is requested and what the patient is told to do when urgent help is needed.

Patient-reported outcomes can add context, but only when chosen deliberately

A support program may collect pain, fatigue, function, quality of life, treatment burden or other patient-reported outcomes. These measures can help a program understand what patients experience between appointments.

FDA's patient-focused drug development guidance recommends selecting or developing clinical outcome assessments that are fit for their intended context of use. Review the final FDA guidance on fit-for-purpose clinical outcome assessments.

A short weekly check-in used to guide routine support is not automatically a research endpoint. A validated PRO instrument collected under a protocol is not automatically suitable for every commercial support program. Define the purpose first, then choose the measure and frequency.

Privacy follows the relationship and the data flow

A manufacturer, hub provider, specialty pharmacy, clinic and app vendor may each play a different role. The program should document which organization collects which data, under what authorization, for what purpose and with which permitted recipients.

Not every health app is governed by HIPAA in the same way. Consumer health products outside HIPAA can still fall under Federal Trade Commission privacy, security and breach-notification requirements. The patient should be able to tell the difference between treatment support, clinical care, promotional communication and research. Data collected for one purpose should not quietly become a different program simply because it is technically accessible.

Use AI for a bounded job

AI may help organize free-text messages, prepare a timeline for a nurse or coordinator, select from approved content or identify incomplete records. Those uses can reduce repetitive work.

It should not alter the prescribed treatment, invent a reason for missed therapy, determine adverse-event seriousness or create individualized medical advice without an appropriate governed and evidenced function.

A sensible program defines the input, output, approved content, escalation and responsible reviewer before the model is added.
