This mood tracker printable is for anyone who wants a clearer picture of how their month really went. You color in each day with your own five color key, mark a score on a chart and tick a few small things like sleep and movement.
Moods shift for many reasons, and a week later they are hard to remember. A page of colors shows your ups and downs at a glance, and the small ticks under each day help you see what your good days share.

- Any five colors will do. Colored pencils work well, and you can write the number from your key instead.
- Fill it in at the same time each day, such as before bed, so you rate the whole day.
- Use the Big days lines for the highs as well as the lows. Good days hold clues too.
- Pick one question to watch each month, like whether walks help, so the month has a focus.
Bringing it to your doctor
Bring your colored month and the Looking back page to your doctor or therapist, and write your questions on the For my next appointment card. If you have thoughts of harming yourself or feel in crisis, call or text 988 to reach the 988 Suicide & Crisis Lifeline, or call 911 in an emergency.
Questions
What is a mood chart?
It is a simple record of how you felt each day, usually as a score or a color. This sheet gives you both, a colored calendar and a 1 to 10 chart you join with a line.
What moods should I put on a mood tracker?
Start with five, from your best to your hardest, such as Great, Good, Okay, Low and Very low. Name them in your own words so each color means something to you.
Is this mood tracker free?
Yes. This free mood tracker printable downloads as an A4 or US Letter PDF. You can print it or fill it in on a computer, and reuse it as a mood tracker template each month.
Can I use it as a daily mood tracker?
Yes. Each day takes about a minute. Color the square, tick the small icons and mark one dot on the chart.
Can I use a mood tracker for bipolar disorder?
Yes, as a record to share with your care team. A month of daily colors and scores gives your doctor a clearer view than memory alone. Keep it alongside any mood diary or plan you already use.
Is there a mood tracker printable for kids?
Yes. Children can use this one with a little help, since coloring a square is easier than describing a feeling. An adult can help them name the five colors.
