This doctor appointment checklist is a free printable for anyone who wants to walk into a visit prepared and walk out with a clear plan. It works for a family doctor, a specialist or a follow-up, and you can fill it in on your phone or on paper.
Visits are short, and it is easy to forget the one thing you came for. Writing it down first means it gets said. Writing the answers down means they come home with you, as doctor appointment notes you can read again later.

How it helps
Say the one thing that matters
A box at the top of the first page holds the one thing you need from this visit, so it gets covered even if time runs short.
Give your doctor a clear picture
A short timeline of changes and a symptom snapshot show what is new, how often it happens and how much it affects your day.
Leave with a plan you can follow
Space for tests, medicine changes, next steps and when to call back keeps the advice from each visit in one place.
How to use it

- During Read from the page, then note what the doctor said, any tests or referrals, and any medicine that is new, changed or stopped.
- After Check whether you got your one thing. If not, write your next step, then add the visit to your list of visits.
What is inside
- Before the visitA filled example to copy, then a blank page for your one thing, changes, symptoms, questions, medicines and what to bring.
- During and afterWhat the doctor said, tests or referrals, medicine changes, follow-up and when to call back.

See a filled example
The example is a visit with Dr. Lee, a family doctor, on Thursday, March 12, about tiredness and headaches. The one thing is to find out why the tiredness never lifts and whether blood tests need repeating. The timeline shows amlodipine 5 mg started on February 1 and headaches went from once a week to most days by February 17. That is why the second question asks whether the new blood pressure tablet could cause headaches.

- Print two copies. Keep one for your notes and hand the other to your doctor at the start of the visit.
- Put your most important question first. If time runs out, the top one still gets asked.
- Add a date to every change. Headaches most days since mid February is easier to act on than more headaches lately.
- Tick the Bring with me list the night before, so your ID, insurance card, medicine list and test results are ready to go.
Bringing it to your doctor
Hand your doctor the Before the visit page at the start, so you both work from the same list. Before you leave, use the When to call back card to write down which signs mean you should call, and the phone number to use. If something feels urgent before the visit, call your doctor's office or get urgent care right away.
Questions
How do I prepare for a doctor's appointment?
Write down the one thing you need from the visit, what has changed since last time, and your top questions. Add your current medicines with doses. Do it in the days before, when you have time to think, and bring the page with you.
What should I bring to a doctor's appointment?
Bring your ID and insurance card, a list of your medicines and doses, and any recent test results. A support person can help you listen and take notes. The Bring with me list on the first page covers the basics.
What should I ask my doctor?
Ask about what worries you most, starting with the most important. Specific questions get clearer answers, such as whether a new medicine could cause a symptom or what a test result means for you. The first page has room for your top 3.
What should I tell my doctor?
Tell your doctor what has changed, when it started, how often it happens and how much it affects your day. Mention what helps, and every medicine or supplement you take. The symptom snapshot covers each of these in one row.
How do I keep track of doctor's appointments?
Keep one line per visit with the date, who you saw, what you needed and the next step. The My visits at a glance page does this, so you can see what got sorted and what still needs a push. You can also keep your visits and notes in the CareClinic app on your phone.
Is there a free doctor visit checklist PDF?
Yes. This doctor visit checklist PDF is free in A4 and US Letter sizes. You can fill it in on a phone or computer, or print it and use it as a doctor appointment template for every visit.
