What is a fever?

The science behind fevers

Body temperature is regulated by the hypothalamus in the brain. Pyrogen triggers fever and results in the release of prostaglandin E2 (PGE2). PGE2 acts on the hypothalamus and creates a systemic response in the body which raises your overall temperature and set point. Constriction of peripheral blood vessels reduces heat loss through the skin and causes the person to feel cold. Muscle contraction occurs through shivering and raises your body’s rate of metabolism.

When the hypothalamic set point moves back to baseline—either spontaneously or via medication—normal functions such as sweating, vasodilation and the stoppage of shivering are used to cool the body to the new, lower set point for temperature.

Types of fever

Different types of fever can indicate the potential cause behind the fever and lead to a diagnosis. The different types of fever are listed and described below:

  • Continuous fever is when your body temperature remains above normal throughout the day and does not fluctuate more than 1 °C in 24 hours. This may indicate pneumonia, typhoid, meningitis or urinary tract infection.
  • Intermittent fever is when your body temperature is above the normal set point for a little while before going back to normal. This may indicate malaria, leishmaniasis or sepsis.
  • Remittent fever is when your body temperature remains above normal throughout the day and fluctuates more than 1 °C in 24 hours. This an indicate infective endocarditis or brucellosis.
  • Malaria can cause quotidian fever, tertian fever and quartan fever.
  • Neutropenic fever occurs when your immune system is not functioning normally. This is considered a medical emergency and is commonly seen in patients receiving chemotherapy which suppresses your immune system.

Fever of unknown origin is the term used to describe persistent fever that cannot be explained after repeated routine clinical inquiries.

  • Between ages 6 months and 2 years old and has a rectal temperature higher than 102 F (38.9 C) that lasts longer than one day but shows no other symptoms.
