What is a fever?
Fever or pyrexia is when you have a temperature that is above the normal range or body’s set point. The normal body temperature is usually between 37.2 and 38.3 degrees Celsius or 99 and 100.9 degrees Fahrenheit. When your body temperature moves above its normal set point, this causes your muscles to contract, causing you to feel cold. This results in greater heat production and efforts to conserve heat. When the set point temperature returns to normal, you feel hot and become flushed. You may also begin to sweat. Young children may also experience a febrile seizure but this is very rare.
What causes a fever?
A fever can be caused by a variety of medical conditions which can be non-serious or life-threatening. Fevers can result from infections by viruses, bacteria or parasites. Examples include influenza, meningitis, urinary tract infections (UTIs), appendicitis and malaria. Fevers can also result from vasculitis, which is when your blood vessels become inflamed. Other non-infectious causes include deep vein thrombosis (DVT), connective tissue disease and cancer.
The science behind fevers
Body temperature is regulated by the hypothalamus in the brain. Pyrogen is triggers a fever and results in the release of prostaglandin E2 (PGE2). PGE2 acts on the hypothalamus and creates a systemic response in the body which raises your overall temperature and set point. Constriction of peripheral blood vessels reduce heat loss through the skin and causes the person to feel cold. Muscle contraction occurs through shivering and raises your body’s rate of metabolism.
When the hypothalamic set point moves back to baseline—either spontaneously or via medication—normal functions such as sweating, vasodilation and the stoppage of shivering are used to cool the body to the new, lower set point for temperature.
Increased heart rate and vasoconstriction contribute to raised blood pressure during a fever.
Types of fever
Different types of fever can indicate the potential cause behind the fever and lead to a diagnosis. The different types of fever are listed and described below:
- Continuous fever is when your body temperature remains above normal throughout the day and does not fluctuate more than 1 °C in 24 hours. This may indicate pneumonia, typhoid, meningitis or urinary tract infection.
- Intermittent fever is when your body temperature is above the normal set point for a little while before going back to normal. This may indicate malaria, leishmaniasis or sepsis.
- Remittent fever is when your body temperature remains above normal throughout the day and fluctuates more than 1 °C in 24 hours. This an indicate infective endocarditis or brucellosis.
- Malaria can cause quotidian fever, tertian fever and quartan fever.
- Neutropenic fever occurs when your immune system is not functioning normally. This is considered a medical emergency and commonly seen in patients receiving chemotherapy which suppresses your immune system.
Fever of unknown origin is the term used to describe persistent fever that cannot be explained after repeated routine clinical inquiries.
Why use a fever tracker?
It is important to closely monitor your body temperature if you are experiencing a fever. This is because a fever can indicate an underlying issue that may be serious. Fevers may not be harmful themselves but it is important to visit a doctor under specific circumstances, especially when your child is experiencing a fever.
A fever of unknown origin is greater cause for concern in infants and in children than in adults. It always important to talk to your doctor if your child is experiencing a cold or diarrhea along with their fever. Consult your child’s doctor if your child is:
- Younger than 3 months and has a rectal temperature of 100.4 F (38 C) or higher.
- Between ages 3 and 6 months and has a rectal temperature up to 102 F (38.9 C) and seems unusually irritable or uncomfortable
- Between ages 6 months and 2 years old and has a rectal temperature higher than 102 F (38.9 C) that lasts longer than one day but shows no other symptoms.
Common adult fever symptoms are the following:
- warm forehead
- aching muscles
- general feeling of weakness
- sore eyes
- loss of appetite
- swollen lymph nodes
In early 2020, Covid-19 started making headlines for being a new virus that included fever like symptoms. Covid-19 escalates symptoms until it is much worse than a common flu. Many hospitals during this time have recommended staying home and tracking your symptoms. Tracking your symptoms can be hard when you have a headache, chills, and a dry cough so you can easily use an app like Care Clinic.
Treating a fever
Depending on your age, physical condition, and the underlying cause(s) of your fever, you may or may not require medical treatment from your healthcare professional. When fevers occur, they are usually your body naturally trying to defend itself against infection. However, a fever is usually associated with physical discomfort and thus, it is important to be aware of the various ways to reduce your fever.
Treatments vary depending on the cause of the fever. For instance, antibiotics would be prescribed for fevers caused by bacterial infections. The most common treatments include over the counter drugs like acetaminophen and NSAIDs.
If you are experiencing a fever, it is important to do the following:
- Drink a lot of clear liquids such as water, broth, and juices. This is to ensure that you stay hydrated. It might even be beneficial for you to track the liquids you are consuming.
- Take a lukewarm bath.
- Rest and stay home.
- Keep yourself cool with lightweight clothing and bed coverings.
